Stuck Thinking & How to Get Unstuck: The 5 Questions That Retrain Your Nervous System

Have you ever noticed how a single thought can hijack your entire day?
You wake up feeling fine…and then it happens.

“I’m never going to get back to who I was.”
“This isn’t working.”
“I always mess this up.”
“I’m too far gone.”
And just like that, your chest tightens.
Your breathing shifts.
Your mood drops.
Nothing external changed.
It was just a thought and your nervous system believed.
When teaching HRV Biofeedback, my clients and I spend time talking about something called stuck thinking. Patterns of thought that occur without conscious action or intent. Here’s the thing, our brains are efficient — and sometimes your brain can become efficient at repeating patterns that no longer serve you.
But here’s the powerful part, and what opens the door for so many of my clients: Thoughts influence emotion and physiology. Emotions influence physiology, and cues from our physiology get interpreted by our brain, often as alarm, and the cycle is underway.
Enter a technique that allows an individual to quiet and calm physiology at will, shift emotional states with ease, and test those automatic thoughts to see if they are true to interrupt detrimental negative thought loops.
Automatic Negative Thoughts (ANTs)
Most of the thoughts that drive our emotional reactions aren’t carefully chosen. They’re automatic.
Fast. Convincing. Often dramatic.

“I’ll never get better.”
“They’re judging me."
“This always happens.”
“It’s hopeless.”
Dr. Daniel Amen calls these Automatic Negative Thoughts — or ANTs — and also describes the “dragons from your past,” the old narratives and emotional wounds that shape how we interpret the present. These questions are drawn from Dr. Daniel Amen’s work in Your Brain Is Always Listening and his Brain Health Certification training, and I’ve integrated them into our HRV Biofeedback framework at CBFC.
I don’t teach them as standalone cognitive tricks. I teach them alongside nervous system training. Because when you pair perspective work with physiology, something shifts on a much deeper level.
The 5 Questions
Here are the five questions we use:
Is this thought true?
How can I be absolutely sure it’s true?
How do I feel when I have this thought? How do I behave?
Could the opposite thought be even more true?
And then the question that changes everything:
5. Who would I be without this thought?
These questions aren’t about forced or toxic positivity. They’re about loosening rigid certainty and exchanging instead for accurate thinking.

Walking Through an Example
Let’s take a thought that often shows up after trauma, concussion, anxiety recovery, burnout, or illness:
“I am never going to get back to who I was.”
Sound familiar? That thought feels heavy. Final. Absolute. So let’s test that thought. We don’t have to believe everything we think, because not all thoughts are true.
Question 1: Is this thought true?
Yes! That’s the automatic answer, isn’t it? Or maybe you know cognitively that this thought isn’t true, but that doesn’t stop it from feeling true, right?
This is what we need to remember: feelings aren’t always facts. So let’s keep testing.
Question 2: Can I be absolutely sure that this thought is true?
“I am never going to get back to who I was.”
How do you know? Can you predict the future? Has healing never happened in your life before? Have you ever grown in ways you couldn’t have imagined? Already, something softens.
Question 3: How do I feel when I have this thought? How do I behave?
Hopeless. Defeated. Anxious. Heavy.
And how do you behave? Do you withdraw? Brace? Maybe even stop trying?
Now I want you to pause and notice something powerful:
Notice how that one thought has cascaded into your emotions. Those emotions then spark a change in your physiology. And now your physiology is reinforcing the thought. That’s the feedback loop you’re stuck in right now.
Where HRV Biofeedback Changes Everything

Question 4: Could the Opposite Thought Be Even More True?
What if, instead of “I am never going to get back to who I was,” it turned into:
“I’m learning everything I can from this experience and leaving the rest behind. The person who steps forward is more than my old self. It’s stronger, wiser, more balanced.”
Or: “My nervous system is healing, even if it’s slow.”
Or: “I am still who I was, and I am more than who I was.”
This isn’t denial. It’s possibility, a
nd possibility changes physiology.
Question 5: Who Would I Be Without This Thought?
This question touches identity. Without the thought “I am never going to get back to who I was,” who would you be? Not what would you do, but who would you be?
Notice what happens in your body as you consider that question. Often there’s a subtle softening — a little more space in the chest, a little less tension in the shoulders. The feeling of something rising within.
Who would you be?
More open? More patient? More willing to try? Less tense? Less frustrated or afraid?
That space opening in your chest as you contemplate this question?
That’s the beginning of regulation, and regulation is healing. One pass through these questions is rarely enough to silence a long-standing ANT. The brain wires through repetition — and it unwires the same way. One pass, though, is often enough to experience the shift, and want to reinforce it again.
Thoughts, Emotion, and the Nervous System

When we believe a threatening thought:
The amygdala activates
The sympathetic nervous system revs
Heart rhythm becomes erratic
HRV drops
Cognitive flexibility decreases
When we pause and shift our breathing first:
HRV increases
Vagal tone strengthens
Emotional reactivity decreases
New thoughts become accessible
You’re not forcing yourself to think positively, but more accurately. You’re training your nervous system so that flexible thinking becomes possible. What is resonant breathing frequency ?
Your resonant breathing frequency is the rate of breathing at which your breath and your heart rate synchronize so that as you inhale, your heart rate is rising, and as you exhale, your heart rate is falling. It’s important because it aligns breathing with the body's cardiovascular system, maximizing Heart Rate Variability (HRV) and optimizing baroreflex gain. This coherence between heart rate and respiration reduces blood pressure, lowers stress, enhances autonomic balance, and improves cognitive function.
